Congratulations on enrolling in our Healthy Heart Marketplace plan!

Now that you have your plan, start right by making an appointment with your in-network family doctor and/or cardiologist. If you don’t have a provider that is in-network, use our Find a Doctor tool to find one now.

  1. Take the brochure that you got in your welcome kit with you. If you don’t have it, you can view or download it Navigate here.
  2. As a Healthy Heart plan member, you get many drugs, services, and supplies at $0 cost to you!
    • If you need any of the $0 cost tests available, such as an electrocardiogram, get them.
    • If you are on any of these $0 cost drugs, such as digoxin or Entresto* and if they are appropriate for your condition, ask your provider for a new prescription.
    • If you need a blood pressure cuff or compression socks, ask your provider to write a prescription for them and take it to a medical equipment supplier. You can find these suppliers listed on our Find a Doctor tool. Just filter for Type: Medical Equipment.
  3. Sign-up for home delivery of your maintenance medications. It is a convenient way to have a 90-day supply of medicines delivered right to your door! Ask your provider to ‘e-prescribe’ a 3-month supply to Express Scripts. Your provider should already have the website and phone number.
