Early & Periodic Screening, Diagnostic & Treatment Wellness Visit
It’s recommended that teens and young adults visit their primary care provider (PCP) for a Healthchek exam every year up to age 21. Healthchek is Ohio’s EPSDT benefit. EPSDT covers care for those under the age of 21 at no cost to you. Your child’s primary care provider (PCP) will work with you to make sure your child gets the care they need. Healthchek exams are offered at $0. They include:
- a review of health since birth including growth and developmental milestones
- a full exam
- eye testing
- a dental exam
- hearing exam
- a check of shot records
- other tests as needed
*Some of these tests and treatment services may require approval or prior authorization from your child’s provider.
