If you offer medical services and want more information about becoming a participating provider, please submit the New Health Partner Contract Form.
FILL OUT AN APPLICATION
Once you submit your application, you will receive a confirmation email. Please save this email, as it will contain your Application ID.
You can check your application status on the Provider Portal. You will need to enter your NPI and Application ID to view your status.
Step 2: Get Credentialed.

Providers who wish to contract with CareSource® must also be credentialed with the Ohio Department of Medicaid Centralized Credentialing program.

Through credentialing, we check the qualifications and performance of physicians and other health care practitioners. Providers are recredentialed a minimum of every three years and are considered to be recredentialed unless otherwise notified.
Contracting is the process of the provider and Managed Care Entity (MCE) formally executing an agreement for the provider to deliver medical services that outlines reimbursement rates, scope of services, etc.

It is the policy of CareSource to ensure the quality and qualifications of professionally licensed practitioners and organizational providers through a credentialing and recredentialing process that complies with regulatory and accreditation standards. Our Vice President/Senior Medical Director is responsible for the credentialing and recredentialing program.
You must submit a complete Council for Affordable Quality Healthcare (CAQH) application or CAQH number and National Provider Identifier (NPI) number to CareSource.

CareSource is committed to a seamless and expedient credentialing and re-credentialing process.

You can check your application and credentialing status on the Provider Portal. You will need to enter your NPI and Application ID. The tool will allow you to view your status in the credentialing process as well as specific credentialing documents.
Once we complete the credentialing process, you will receive a letter from us.
Step 3: Finish Onboarding.

Welcome to the CareSource network! Once you receive your Welcome Letter from CareSource with your CareSource ID number, you have completed the contracting and credentialing process.
As you prepare to see CareSource members, we encourage you to familiarize yourself with our plan, tools and resources.
If you are not registered with CareSource’s Provider Portal, please follow these easy steps:
- Visit the Provider Portal. Click “Sign Up” to establish your account by creating your username and password.
- For added security, set up the multifactor authentication.
- To connect your account, you will need your Provider Name, Tax ID, Provider ID and your Zip Code.
- Review and accept the Agreement.

CareSource MyCare providers are required by CMS to attest to completing the annual model of care training. To attest that you have completed the training and receive credit, please log on to the Provider Portal, which will prompt you to review and attest to completing the model of care training.

CareSource has partnered with ECHO Health, Inc. to deliver provider payments. ECHO offers three payment options:
- Electronic fund transfer (EFT) – preferred
- Virtual Card Payment (QuicRemit) – Standard bank and card issuer fees apply*
- Paper Checks
*Payment processing fees are what you pay your bank and credit card processor for use of payment via credit card.
Enrollment Instructions
Enroll with ECHO for payment and choose EFT as your payment preference for CareSource. You can also complete the ECHO enrollment form and fax, email, or mail it back to ECHO.

Attention Ohio Medicaid and MyCare Providers:
Effective Oct. 1, 2022, Ohio Medicaid and MyCare providers will utilize the Provider Network Management (PNM) module from the Ohio Department of Medicaid for submitting provider applications, credentialing request and provider demographic updates. The PNM module will be the single point for providers to complete provider enrollment, centralized credentialing, and provider self-service. The PNM module is replacing MITS provider portal.
