Waiver Services
Waiver services are person-centered services to help you meet your daily needs. You may qualify if you are:
- 18 years or older.
- Fully eligible for both Medicare and Medicaid.
- Enrolled in a MyCare Ohio plan.
- Meet an intermediate or skilled level of care as determined by the State of Ohio rules.
Waiver services include a variety of services including:
- Personal aide services
- Home delivered meals
- Chore services
- Home modification
With waiver services, you will have a Care Manager who approves services based on your needs using a Service Plan. It shows the name of the agency or person who will provide each service. It also shows how often you get the service. You can access your service plan through your My CareSource account or you can print a copy. Each time the service plan is updated by a care manager, you will receive a copy.

Incidents
If the care you get ever causes you harm or makes you afraid, this is called an “incident.” To learn more about how to report incidents, please see your Waiver member handbook on our Plan Documents page. You can report any incidents or concerns to your Care Manager. You can also call Member Services at 1-855-475-3163 (TTY: 1-833-711-4711 or 711), 8 a.m. to 8 p.m., Monday through Friday.
