HOW TO CHOOSE CARESOURCE
Medicaid health care coverage is available for eligible Ohioans with low income, pregnant women, infants and children, older adults and individuals with disabilities. CareSource Medicaid is available across the state of Ohio.

When you apply for Ohio Medicaid, you can choose CareSource as your managed care plan. Whether you apply online, by mail, over the phone or at your local Department of Job and Family Services (JFS), tell them you want to choose CareSource. |
| Switch to CareSource |

If you were assigned to another managed care plan and want to switch to CareSource, call the Ohio Medicaid hotline, complete a change request within 90 days and tell them you want to switch to CareSource. If you have been on your plan more than 90 days, you can switch to CareSource during your next open enrollment period. |

Apply online using the self-service Ohio Benefits Portal, by calling the Ohio Medicaid Consumer Hotline at 1-800-324-8680 or by visiting your local Department of Job and Family Services (JFS) office.

If you get assigned to a health plan other than CareSource, you can switch within 90 days by calling the Ohio Medicaid Consumer Hotline at 1-800-324-8680.
Ohio Medicaid annual open enrollment
Ohio Medicaid’s annual open enrollment is happening now through November 30! Members are encouraged to review the managed care plans available and select the one that best fits your healthcare needs.
If you would like to keep your current managed care plan, you don’t have to do anything.
If you decide to switch, coverage by your newly selected plan begins the first day of the following month. For example, if you select a new plan in November and its coverage begins December 1.
Taking part in annual open enrollment allows you to focus on you and your family’s well-being. While reviewing plans, keep the following in mind:
- Which plan works with all or most of your doctors?
- Which plan works with the hospitals you want to use?
- Which plan offers the extra services you need, such as additional unemployment assistance, access to nutritious foods, education, wellness programs, vision, or dental services?
- Will you have to pay co-pays for dental services, routine eye exams, eyeglasses, mental health, substance use disorder benefits, or non-emergency services provided in a hospital emergency department?

Representatives can provide interpreters and answer questions about open enrollment and are available 7 a.m.-8 p.m. Monday through Friday and 8 a.m.-to 5 p.m. Saturday, Eastern time.
