End or Change Membership

We hope you will be happy with CareSource. If you have problems or concerns about your benefits, please call us so we can try to resolve them with you. Because the Community Transition Program is a temporary benefit most members will see their benefits automatically end after one year. When you have come to the end of your benefit period, a letter will be mailed to the address we have on file and copied to your assigned CTP provider agency. However, you do have the right to end your enrollment in CTP at any time.
