Select a Primary Care Provider (PCP)
When possible, your primary care provider (PCP) should be your first choice for care. You can choose a new PCP by logging into your My CareSource® account and selecting one that is in the CareSource network.

Note:
- If you do not choose a PCP within 30 days of enrollment, one will be assigned to you.
- Your PCP is listed on your CareSource member ID card. You will get a new card anytime you choose a new PCP. Your new card should arrive in the mail within 10 days.
CareSource works with certain doctors and providers to get you care. We call these in-network providers. To have your health care services covered by your CareSource plan, you must go to an in-network provider. Neither Medicare nor CareSource will be responsible for the costs unless you get approval from CareSource. You should use an in-network pharmacy to get your prescriptions unless there is a non-routine circumstance where you cannot use one. The Comprehensive Formulary, pharmacy network and/or provider network may change at any time. You will be notified when necessary.
You must use the in-network providers except in emergency or urgent care situations. If you obtain routine care from out-of-network-providers, neither Medicare nor CareSource will be responsible for the costs, unless specifically authorized by CareSource. You must use in-network pharmacies to access the prescription drug benefit, except under non-routine circumstances when you cannot reasonably use an in-network pharmacy.
