How Care Management Helped Teresa with her Pregnancy

When CareSource member Teresa found out one of the twins she was pregnant with had Down syndrome, her CareSource care manager was there to help every step of the way.

CareSource offers free care management services to children and adults. The care management team includes registered nurses, social workers and other outreach workers called Care Managers. Care Managers work with our members, their primary care provider (PCP) and other specialists and any family or caregivers to help coordinate care. Together, we work with our members to meet their health and wellness goals and improve outcomes.
