Transition of Care

If you’re a new HAP CareSource member, you can keep your out-of-network doctors and services for at least 90 business days. 

Please call Member Services at 1-833-230-2053 (TTY: 711) to request keeping your out-of-network doctor for 90 days as part of Transition of Care service if you have the below conditions, or you think your condition requires this service.

You can keep seeing your doctor if you’re getting care for certain chronic conditions. 

Some examples of approved on-going care: 

  • Chemotherapy and radiation therapy
  • Hemodialysis
  • Second and third trimester pregnancy 
  • Home infusion 
  • Acute care hospitalization
  • Skilled nursing care
  • Inpatient rehab
  • Long-term and post-acute care
  • Surgeries scheduled within 30 days
  • Post-surgical follow-up
  • Medications
