Foster Care

HAP CareSource has a team that helps children and youth in foster care. We support the children, their caregivers and foster agencies. All members are assigned a Care Manager to help you understand your health care needs.

The Health Care Transition (HCT) program helps youth aging out of foster. Your Care Manager can:

  • Help with the Transition Readiness Assessment Tool. This tool shows areas where you may need extra help on your health care transition.
  • Teach you about the privacy and consent changes when you turn 18.
  • Help you find adult providers.
