Care & Disease Management
HAP CareSource offers care and disease management programs. These programs can help you learn about your health and how you can better manage your specific health conditions. Our goal is to make sure you have the right tools to stay as healthy as possible. These programs are available to you at no cost.
Care Management
HAP CareSource has nurses, social workers, and community health workers who can work with you one-on-one to help coordinate your health and non-health care needs. They can contact you by phone:
- If your doctor requests one to contact you
- If you request a phone call
- If our Care Management staff thinks their services would be helpful to you or your family
Our staff is trained to help you and your family with any special medical problems like asthma, cancer or with other medical conditions. We can also work with you if you need help figuring out when to get medical care from your doctor, an urgent care center or the emergency room.
Diabetes Coaching Program – A Care Manager can help you learn to manage your diabetes, whether or not you are newly diagnosed. Call your Care Manager or the Care Management line directly.

Disease Management
Living with a health condition can be hard. We have free disease management programs that can help you learn more about your health and better manage your specific health conditions. We want you to have the right tools to stay as healthy as possible. As a HAP CareSource member, you have access to:
- The MyHealth online tool for members 18+. You can participate in a journey to improve your health.
- Materials with helpful tips and information to manage your condition and learn self-management skills.
- Care coordination with an outreach team.
- One-to-one care management
We may receive information from your doctor, pharmacy or other health care source to let us know you might benefit from one of our programs. We will send you materials related to your health condition along with tips and resources available to you. All ages (children, teens, and adults) are eligible to participate. You can self-refer or be referred into a disease management program to receive information about your condition or have someone reach out to you.
