Request a CareSource Member ID Card
If you have lost your HAP CareSource member ID card, or need an additional card, you can easily request one any time through your My CareSource portal account. Click the Request ID Card button on the MyHelp menu to have a replacement sent to you.
You can also call Member Services at 1-833-230-2099 (TTY: 711) Monday through Friday, from 7 a.m. to 7 p.m., Eastern Time (ET) to request a new ID card.
