Fill out your Health Needs Assessment (HNA)!
The HNA is a questionnaire. It asks about your health history and helps us identify health issues or concerns. This lets us know where we might be able to help you get assistance. It only takes a few minutes to complete.
Completing the questionnaire helps us help you lead a healthier and safer life. You can complete the HNA in one of the following ways:
- Over the phone by calling our Member Assessment Team at 1-844-217-1357. This is the fastest and easiest way!
- Online through the My CareSource® Member Portal at My.CareSource.com. Go to the Health tab at the top. Scroll and click on the Health Needs Assessment start button.
- By mail.
My Health
Once you take the HNA, you get a personalized health guide in your private MyHealth account. It will show how you can get healthy and lower your risk for health problems through:
- Nutrition and exercise
- Preventive health
- Emotional health
- Safety behaviors
- Tobacco, drug and alcohol use
- Blood pressure, cholesterol, body mass index (BMI)
You won’t have to wonder where to begin or what to do next — it’s there in your guide. Come back to the tool often to track your progress.
