Health Needs Assessment

Take a Health Needs Assessment (HNA) when you join HAP CareSource and each year after. Answer questions about your health and habits to help you identify your health needs.

You can take the HNA in one of these ways:

  • Online: Take the HNA online though your private My CareSource portal account. To get started, log in to your My CareSource account and select MyHealth
  • On paper: You can request an HNA by calling Member Services. Fill it out and return it in the postage-paid reply envelope.

My Health

Members age 18 and over can get a personalized health guide in their My Health portal account. It will show how to get healthy and lower the risk for health problems through:

  • Nutrition and exercise
  • Preventive health
  • Emotional health
  • Safety behaviors
  • Tobacco, drug and alcohol use
  • Blood pressure, cholesterol, body mass index (BMI)

You won’t have to wonder where to begin or what to do next — it’s there in your guide. Come back to MyHealth often to track your progress.
