Congratulations! Being pregnant is a brand-new stage of your life and it’s normal to have questions.  We are here to help you every step of the way.

We have nurses, social workers and community health workers who can work with you to help coordinate your health and non-health care needs. Programs for You:

Mom and Baby Beginnings

The Mom and Baby Beginnings program has many types of staff to help you have a healthy pregnancy.  We have staff like nurses, socials workers, behavioral health, substance use case managers and breastfeeding specialists. Healthy Mom, Healthy Baby

Having a healthy baby starts before you’re pregnant. Whether you are planning to become pregnant soon or not, it’s key that you take care of yourself now. Once you are pregnant, see your doctor as soon as you can. They will help you learn more about how you and your baby can stay healthy.

NICU Care Management

We have a NICU Care Management program with a team of expert nurses and staff. They work with you to make sure you and your baby have a timely discharge from the NICU. They also support a safe transition home. The NICU Care Manager will build a relationship during your and your baby’s time in the hospital. They help support:

  • Maternal health
  • Postpartum follow-up
  • Assessing your support system
  • Physical, social and behavioral need
  • Collaboration with facilities for discharge planning
  • Identifying your needs for help by the integrated care team.
  • Creating a care plan just for you, to ensure your baby and family have the resources you need.
