Were You Recently Released From The Hospital? Find Out How CareSource Can Help
CareSource offers care and disease management programs. These programs can help you learn about your health and how you can better manage your specific health conditions. Our goal is to make sure you have the right tools to stay as healthy as possible. These programs are available to you at no cost.
Care Management
This team is registered nurses, social workers, and other outreach workers. They are called Care Managers. They work with you, your providers, family, or caregivers to set up your care. We work with you to meet your health goals. This is at no cost to you.
The level of care is tailored to your health care needs. Start by filling out your Health Needs Assessment (HNA). It shows us the level of care for you.
You can take the HNA by:
- Phone: Call 1-833-230-2011 (TTY: 711) between Monday – Friday,7 a.m. to 6 p.m.
- Online: Just log into your secure My.caresource.com account and click on the Health tab.
Care Management can support any medical condition or link to resources. Here are a few of the conditions:
- Asthma
- Babies in the Neonatal Intensive Care Unit (NICU)
- Chronic obstructive pulmonary disease (COPD)
- Diabetes
- ERvisits
- Heart disease
- High blood pressure
- High-risk pregnancy
- Lead poisoning
- Medically frail
- Mental health and substance use disorder
- Pain management
Care Managers will ask you about your health problems. They can help you with your health. They can also help with food, clothing, and housing. They can even help you set up rides to get care.
You may hear from them if:
- Your doctor asks.
- You or your caregiver asks us.
- We think we can help you.

Disease Management
Do you have chronic health issues? This can be diabetes, asthma, high blood pressure or Chronic Obstructive Pulmonary Disorder (COPD).
Our free programs can help you learn more about your health. You can choose to join the program, or your provider will tell us. Call 1-844-438-9498 if you would like join. You can also opt-out by calling this number. We want to help you be healthy and well.
Disease Management for Kids and Teens
Kids and teens may need help if they are dealing with a chronic condition. Visit the Health Care page to find out more.
- Asthma Center: Asthma keeps more kids home from school than any other health problem. Help your child handle it, stay healthy, and stay in school.
- Diabetes Center: Get help to keep your child healthy.

MyHealth: Adults ages 18 and older can get a health tool called Journeys®. With it, you can track workouts and use health tips.
