Want to choose or change your primary care provider (PCP)? Log into your CareSource MyLife account. Choose one of the providers in the network. You can also call us.
Your PCP is listed on your ID card. You will get a new ID card when you choose a new provider. Your new card should be in the mail within 10 days.
You can also choose your Dental Home. Log into your My CareSource account. Choose a dentist. You can also call us.
Your dentist is also listed on your ID card. You will get a new ID card when you choose a new dentist. Your new card should be in the mail within 10 days.
