My CareSource Rewards Program
CareSource rewards its members for taking a proactive role in their health and well-being. We have developed a lifestyle rewards program to encourage members to participate in various annual wellness visits and preventive care programs.
Through the My CareSource Rewards Program, members can earn up to $365 annually for completing healthy activities! The rewards available will vary depending on the member’s age, gender and health needs.
Below is a list of activities you can complete to earn rewards.
CareSource members are automatically enrolled in this program. They receive a My CareSource Rewards Card in the mail along with additional information about the program and details on how to activate the rewards card. As they complete a healthy activity, rewards are added to the card. The My CareSource Rewards Card can be used at Kroger, Walmart, Walgreens and Walmart.com. Some restrictions apply. - If you are no longer a CareSource member, your access to the Rewards Portal will be deactivated and any unused Rewards may be no longer available.

- To check you rewards balance go to www.HealthyBenefitsPlus.com/MyCare
- Your My CareSource Rewards card never expires. Rewards are added to the same card every year. Do not throw your rewards card away at the end of the year. You will not receive a new card every year.
- Card Fees: CareSource members will NOT be charged for replacement cards and IVR fees listed on the back of the rewards card.
- Please note that it can take up to 45-days after the completion of your activity before the reward is added to your card.
- This card is not a credit card but may be entered as “credit” to checkout. If prompted, your PIN is the last 4 digits of your card number.
- This card is not accepted at ATMs and cannot be used to obtain cash.
MyHealth for Adults
CareSource offers a second online wellness program that encourages members to take charge of their health. MyHealth is available to all members at no additional cost. Through MyHealth, CareSource members have access to personalized health tools called Journeys, the ability to track exercise goals, and small step guides that help members manage health and wellness topics specific to their needs.
To get started with the CareSource online wellness program, members simply login to their personal online My CareSource account, click on the Health tab and then select the link for the MyHealth – Wellness Program.
Health Risks Assessment (HRA)
CareSource MyCare is required to complete an annual Health Risks Assessment (HRA) on all members. Care Managers review HRA’s with members to identify possible health issues, housing, education, employment and behavioral health needs.
Members can complete the HRA through their My CareSource account. Once logged into their account they can click on the Health tab in the top navigation bar and scroll to the Health Risks Assessment section of the page.
