My CareSource Rewards Program
CareSource rewards its members for taking a proactive role in their health and well-being. We have developed a lifestyle rewards program to encourage members to participate in various annual wellness visits and preventive care programs.
Through the My CareSource Rewards Program, members can earn rewards for completing healthy activities! The rewards available will vary depending on the member’s health and needs.
Below is a list of activities you can complete to earn rewards.

| Kidney Screening | All Adults Diagnosed with Diabetes (Type 1 or 2) | $25 once per calendar year |
| Health Needs Assessment (HNA) | New Members Only | $25 when completed within the first 90-days of eligibility; $20 annual completion. |
*The rewards available will vary depending on your health care needs. Note that not all reward activities are covered services annually. You may be responsible for the cost if you do not check with CareSource or your primary care provider (PCP) before receiving services.
CareSource members are automatically enrolled in this program. They receive a VISA Gift Card in the mail along with additional information about the program and details on how to activate the gift card. As they complete a healthy activity, rewards are added to the card. Members can redeem your rewards anywhere VISA is accepted. Some restrictions apply. MyHealth for Adults
CareSource offers a second online wellness program that encourages members to take charge of their health. MyHealth is available to all members at no additional cost. Through MyHealth, CareSource members have access to a personalized health tools called Journeys, the ability to track exercise goals, and small step guides that help members manage health and wellness topics specific to their needs.
To get started with the CareSource online wellness program, members simply login to their personal online My CareSource account, click on the Health tab and then select the link for the MyHealth – Wellness Program.
Health Needs Assessment
CareSource asks that all members complete an annual Health Needs Assessment (HNA). Through a few questions about the member’s health and well-being, CareSource can help identify possible health issues, housing, education and employment concerns and we may be able to provide resources to help address these concerns.
Members can easily complete the HNA online through their My CareSource account. Once logged into their account they can click on the Health tab in the top navigation bar and scroll to the Health Needs Assessment section of the page.
Members can also earn dollars to their My CareSource Rewards card. New members who complete the assessment within their first 90-days of enrollment will have $25 added to their My CareSource Rewards Gift Card. All other members can earn $20 for completing their annual assessment.
